Malayan Insurance Vital Cover is a 5-in-1 insurance plan that provides financial assistance in the event of contraction of

1. Covid-19
2. Dengue
3. Chikungunya
4. Accidents

Eligible age: 1-75 years old

• Loss of Life due to Covid-19: Lump-sum cash assistance benefit following death due to COVID-19 infection
• Daily Hospital Income Benefit: Fixed daily cash benefit for every day of hospital confinement due to COVID-19, up to a maximum of fourteen (14) days,
• Accidental Death: Loss of life arising from an accident, a sudden and unforeseen violent event that results to bodily injury or loss of life.
• Medical Reimbursement: Provides reimbursement for actual expenses (as specified) irrecoverable from other means for the treatment of injuries sustained in an accident.
• Loss of Life due to Dengue & Chikungunya: Offers protection and will pay lumpsum cash assistance benefit up to PHP 150,000

• How many Vital Cover policies can I buy?

An insured person will be covered by a maximum of one (1) policy per any given time

• Are there any occupational restrictions with Vital Cover?

None. Anyone, including frontliners (doctors, nurses, tanods, etc.) may avail Vital Cover.

• How can I avail of MediMate's 24/7 services?

Just call the designated hotline number (landline and mobile) indicated in the policy.

• What are the requirements needed to make a claim?

All required documents must be submitted with a copy of Malayan's Vital Cover insurance policy and a valid government-issued ID:

• COVID-19
  • Loss of Life - Positive Swab Test Result, Medical Certificate, Death Certificate
  • DHIB - Positive Swab Test Result, Medical Certificate and Proof of Confinement
  • Medical Facility/Hospital/LGU or DOH Quarantine Facility
• DENGUE AND CHIKUNGUNYA
  • Loss of Life - Serology Report showing positive sign of infection, Medical Certifi
  • Death Certificate
• ACCIDENT INSURANCE
  • Accidental Death - Death Certificate, Post-Mortem report (if insured died in a hospital),
  • Police Report (if needed), Autopsy report (if needed)
  • Medical Reimbursement - Original Receipts, Medical Certificate, Police Report (

EXCLUSIONS:

1. Infection not medically diagnosed by a licensed physician
2. Claim for Cash Assistance benefit not medically supported
3. Infection contracted within the waiting period
4. Covid-19 illness that manifested and/or treated outside the Philippines.
